Whether you need post-surgical care or help coping with a newly diagnosed illness, we will help make your transition to home care as smooth as possible. Once you are at home, our team of registered nurses, rehabilitation therapists, social workers or home health aides will work closely with your physician to provide care that meets your individualized needs.

In-Home Medical Services
Based on your condition, your care may include:

Skilled nursing care provided by registered nurses (RNs) who assess patient needs, perform skilled medical tasks (such as wound or infection care), and provide education.

Physical therapy to treat an injury with the use of pain management techniques such as exercise, heat and light therapy, therapeutic massage, and equipment training.

Occupational therapy to help patients return to everyday activities such as bathing, dressing and preparing meals.

Speech-language therapy to improve general communication and swallowing difficulties due to illness, injury or stroke.

Medical social work services addresses any social and emotional concerns related to illness and assists with for long-term care planning.

Remote monitoring collects a patient’s health data at home (such as weight, blood pressure, and blood oxygen) and transmits it electronically to the care team. This allows the team to manage the patient’s condition and helps prevent re-hospitalization.

Our services are covered by Medicare, Medicaid, commercial insurance, or private pay plans. We are licensed to provide care to residents living in Monroe County, New York.

Home Health Aides

Our Home Health Aides are certified by the New York State Department of Health and assist with daily activities such as walking, bathing, dressing, cooking, laundry, and light housekeeping. They also provide companionship and ensure patient safety when loved ones can’t be home. Home health aide services may require private payment due to insurance restrictions.

Personal Emergency Response

We offer a variety of affordable 24-hour personal emergency response systems (PERS) that give patients the confidence to live on their own. One press of a small, waterproof help button, worn as a necklace or bracelet, activates the response system. In seconds, you’ll be in touch with a highly trained operator who will determine what kind of help you need.

Home Delivered Meals

Each week, our meal program delivers nutritious meals to our clients’ homes. Our meal service:

Customizes meals to any diet restriction or preferences

Delivers up to two meals per day, which includes
a drink, a freshly-prepared meal, and a dessert
