What is Home Care?

Home Care is care that is provided in the comfort of your home by skilled clinicians under the guidance and oversight of your primary care physician or specialist. It can include:

  • Skilled nursing care for an illness, accident or surgery; wound care; instructions for the self-administration of medications or injections; clinical management of a chronic disease; and monitoring ofna serious illness
  • Physical, occupational and speech-language therapy services
  • Patient and caregiver education for increase independence in managing your health
  • Home Health Aide services to assist with the activities of daily living

How can Home Care help me?

  1. Manage diabetes, congestive heart failure, chronic obstructive pulmonary disease (COPD). Signs and symptoms to report, life style changes, education on new medications
  2. Wound care
  3. Symptom control (Including palliative care) for pain, nausea, vomiting
  4. Anxiety related to the disease, or treatments such as chemotherapy and radiation
  5. Post- hospital care for disease process (sepsis, pneumonia, or acute illness)
  6. Incision care
  7. Drain care
  8. After-care for surgery for reconditioning and restorative care
  9. After-care for surgical and non-surgical fracture repair (ORIF)
  10. Anti-coagulant care and teaching after surgery or accident
  11. Stroke
  12. G-tube care and feedings
  13. IV antibiotics, TPN , IV medications (Lasix)
  14. Home safety (including an evaluation for the placement of durable medical or therapy equipment, the installation of grab bars, and the removal of clutter)

Do you work with my doctor’s office?

We communicate frequently with your physician throughout your episode of care. Additionally, we are connected to the Rochester Regional Health system so you always have a consistent and reliable continuum of care.

Are costs covered by insurance?
All insurance plans have eligibility requirements that need to be met in order for your insurance plan to cover home care services. Our team of experts will verify your benefits to determine your eligibility, and let you know if there are any out of pockets costs.
